A Health care proxy is also referred to as a health care agent or a Power of Attorney for Health Care who will make decisions about your medical care if you become unable to voice your own desires. Choosing a health care proxy is on an individual basis but one who will honor your beliefs, religious tenets and integrity. It’s never too early to name a healthcare proxy once you turn eighteen years of age. Your health and dignity should be discussed with the person you intrust with your life. This person should be able to make difficult decisions at an inopportune time. It’s a matter of life or death.

2. Consultation
Once an initial contact is made, a care representative will reach out to you to gather information and develop a free personalized care plan. The care plan consists of reviewing the level of care needed, reviewing care receivers home, and billing.
3. Enrollment
Once a personal care plan is completed, we will enroll you into our care and match a medical or non-medical caregiver to your case. We will host an interview so that both the care receiver and giver are compatible.
