Our in-home care service is focused on providing care and support to patients with chronic illnesses, like Alzheimer’s, Dementia, and other special conditions. We collaborate with the patient’s healthcare provider/s to develop a customized care plan based on the patient’s needs. Our goal is to provide individuals diagnosed with disorders like Alzheimers and Dementia with cognitive and physical engagement.

Caregivers will make sure that they adhere to their medications and stay safe within their homes, reducing chances of wandering, wounding, and getting lost due to memory problems brought about by the disease. It is imperative to understand that diseases such as Alzheimers and Dementia can cause your loved one to act in ways that are not like them. It’s imperative to know that these symptoms are a reflection of cognitive function decline and memory loss, which require skilled care to manage them appropriately and effectively while maintaining the patients dignity.

    Alzheimer can be very stressful for the family and making decisions on in-home care vs nursing home is a major decision. We have well trained caregivers who are familiar with memory loss but more importantly is removing the triggers that exist when changing their environment and routine. 2. Consultation

Once an initial contact is made, a care representative will reach out to you to gather information and develop a free personalized care plan. The care plan consists of reviewing the level of care needed, reviewing care receivers home, and billing.

3. Enrollment

Once a personal care plan is completed, we will  enroll you into our care and match a medical or non-medical caregiver to your case. We will host an interview so that both the care receiver and giver are compatible.
